Full and Organisation/Agency membership terms
I agree with the statement of purposes and wish to become a member/renew my membership of Women's Health Grampians Incorporated for one year, ending after the Annual General Meeting. In the advent of my admission as a member of the service, I shall at all times comply with the rules of incorporation, of the service.
General membership terms
I agree to return the material borrowed from the library by the due date. I understand the library is a valuable resource and replacing or repairing lost or damaged material costs a great deal of money. I understand that if I fail to return books, videos or any other borrowed resources, or the returned resources are damaged, I will be liable to be invoiced for the replacement or repair cost of the items.
